Disclaimer and Waiver of Liability
I hereby acknowledge that I have voluntarily applied to participate in dog traing activities with K9s in Motion, Inc. I am aware that there are inherent risks and hazards involved with and around dogs and I am vountarily participating in theses activities with the knowledge of these potential dangers. I am not relying on K9s in Motion, Inc to prevent such occurances. In order to participate in K9s in Motion, Inc. training classes, demonstartions or other activities, I, being fully informed of such risks and hazards, agree to assume all risk of such occurance. I hereby waive any and all claims or actions I or my guardian, representatives, or assigns may have against K9s in Motion, Inc. or Dana Pike and agree to release K9s in Motion, Inc and Dana Pike from liability in any and all personal injuries to myself, my dog, children in my charge, or harm to property casused directly or indirectly by any acts that might occur in conjunction with dog training classes or other activities. I also agree to assume sole responsibility for injury or damage caused by myself, children in my charge, or by the dog I own or handle during participation in K9s in Motion, Inc classes, events and activities and further indemnify, defend and hold K9s in Motion, Inc and Dana Pike harmless from any damage, loss, liability, or expense, including legal costs and attorney fees, which result from damage caused by myself, children in my charge or the dogs that I own or handle.
RELESE OF LIABILITY - I have carefully read this release of liability and fully understand the contents therof. I am aware that this is a release of liability and a contract between K9s in Motion, Inc and myself and I agree to it of my own free will.
